LOCATION

The Chinese Kung Fu Association is located at 3511 NE 74th Ave in Portland, Oregon. The entrance is on the side street, halfway down the block. The door is clearly marked.

GENERAL INFORMATION

TThe Chinese Kung Fu Association uses a belt ranking system for all forms of Wushu. When starting training you must earn your white belt, until that time no belt is worn. The belt ranking system is as follows:
White 1-2, Gold 1-2-3, Green, Blue, Red, Brown, Black
Each belt has varying levels depending on the requirements. All requirements are posted on the school wall for each style and level.
  Each level has specific requirements in both training time and teaching time as well as learning forms and techniques. You will be notified of the time requirements upon receiving your next belt.
  Students are requested to train a minimum of twice a week for both fitness and continuity of training. If you will be gone or must miss a class, please notify Sifu.

POLICIES, RULES & REGULATIONS

The following rules are designed to acquaint new students with the procedures of this school and what is expected of each person. Sifu feels that these regulations will allow him to teach more effectively and assist each student in learning more from the class.

1.    All students are to salute before entering or leaving the training floor.
2.    Respect your instructors, fellow members and yourself. Respect the rights and property of others. Respect the art that you are learning. Do not talk while the instructor is talking. If you finish what you have been told to do and the instructor is busy, do it again. Pay attention when a technique or form is being demonstrated or the instructor is speaking. Don't sit on the floor or lean on the wall at any time unless told to by the instructor.
3.    Students should take their attendance card from the box on the front counter and place it on the counter when you first enter the school.
4.    No student is to interfere with any class in progress that he or she does not belong in. While waiting for your class to start, talk quietly and respect your instructor and students. Keep the guest area clean and neat.
5.    Students should wear the uniform of the school. Please wear (approved) shoes at all times. Make sure your uniform is clean and neat. Sashes should be properly tied and shirts tucked in. Make sure you uniform is straightened before bowing to the instructor. Sashes should not be worn outside the school.
6.    Students should not use weapons or training gear without permission or an instructor present.
7.    If a student comes in late, that student will bow in to the training floor and wait for the instructor’s acknowledgment. Upon the instructor bowing you in, go in to the instructor, and apologize for being late and ask permission to join the class. If granted, then fall in for training.
8.    If a student must leave early, they should let the instructor know before class and why. Then at the appointed time, they should go to the instructor, and ask to be excused.
9.    No gum, food or candy will be allowed on the training floor except during special occasions.
10.    No student will be allowed to train while under the influence of alcohol or narcotics.
11.    No senior or advanced student is to take unnecessary advantage of any junior or beginner in or out of the class. In other words, no unnecessary roughness, excessive or brutal contact.
12.    For safety sake, no necklaces, bracelets, chains, earrings, rings or ornaments are to be worn while training.   
13.    All male students must wear protective cups while training. If glasses are worn, they must be of shatterproof glass.
14.    There are to be no street shoes worn in the training area for any reason.
15.    Students are expected to practice only techniques they are told to do, unless otherwise instructed. Please use the training area only for practicing what you have been taught.
16.    Before leaving the training area, make sure that the area is left as clean and orderly as when you arrived. Pick up paper or other litter. The senior student will assign someone to dust mop the floor at the conclusion of class, please do it as quickly as possible to accommodate the next class.

17.    No student is to demonstrate the art outside the school or class without having permission from the instructor. Carry and practice your weapons outside the school with care. The school is not responsible for injury or damage.
18.    Any student who abuses the art is subject to disciplinary actions.
19.    Students will be expected to control themselves and their techniques at all times. Any student losing his temper will be dealt with by the instructor.
20.    Anyone caught stealing from the school or fellow students will be subject to immediate dismissal.
21.    All dues are to be paid on or before the 5th of the month. Don't let it fall behind. There will be a five dollar charge for anyone over seven days late. If you cannot pay or have a problem, speak to the instructor as soon as possible.
22.    Students are not to engage in freestyle sparring unless the instructor is present.
23.    Students should keep an accurate notebook of all the techniques, charts and drawings. This can be a great benefit to you in the future. Be accurate and share with your fellow students.
24.    Make sure you are at least fifteen minutes early to classes. This time could be used for clearing matters with the office, changing uniforms, etc. Please keep the changing area neat and orderly. Do not leave your clothing or belongings anywhere other then the changing room. Please do not leave belongings in the front Guest area.
25.    If you have any questions, ask an instructor or senior student. If they don’t know, they’ll do all they can to find out. Understand everything you do, if you have a question, ask at the end of class.
26.    A student must train a minimum of twice a week. This is to insure proper growth. Any time a student is not training twice a week, it should be approved by the instructor.
27.    If you have signed a contract with the school for training at a special rate, you are responsible to finish the contract regardless of your attendance. Please speak to the instructor if you have problems fulfilling your contract obligations.

MEMBERSHIP AGREEMENT

All financial obligations of members will be paid promptly upon coming due. If any member becomes delinquent of dues, the school may refuse to permit the Member the use of the facilities. In the event it becomes necessary to institute collection proceedings the Member will be responsible for any collection fees. In case suite or action is instituted to enforce compliance with any of the terms or conditions of this agreement or to collect such sums which may become due hereunder, or any portion thereof, the losing party agrees to pay such sums as the trial court may adjudge reasonable as attorney's fees to be allowed to the prevailing party. Member understands that the failure to use the membership and utilize the facilities does not relieve the Member of liability for payment, regardless of circumstances.

Member acknowledges that he has received herewith a complete written statement of all the rules of Chinese Kung-Fu Association. Chinese Kung-Fu Association reserves the right to change all regulations, policies and services to be rendered at any time and without prior written notice to Applicant. Applicant (s) shall not assign this Agreement, nor any part thereof, without the express prior written consent of Chinese Kung-Fu Association .

Member understands there are certain inherent dangers in the study and practice of kung fu and other martial arts and injuries may occur regardless of precautions taken. Member understands the use of sparring equipment, specially fitted mouthpieces, and groin cups for male participants, are MANDATORY. Member understands that even if he or she uses sparring equipment, specially fitted mouthpieces, and groin cups, injury may be unavoidable. Member assumes sole responsibility to obtain and use protective equipment.

Member assume all risks, dangers and injuries he or she may suffer arising out of his or her further instruction and participation in Kung Fu, self defense and/or other martial arts and agrees never to bring any claims or legal proceedings, suits or other actions against the School, its franchises, employees, teachers, instructors, management, or classmates, because of injury to him/her or damage to his/her property that he or she may suffer. Member understands that from time-to-time he or she will be engaged in unsupervised activity by himself or herself, with other students of school and franchisees of such school and with other persons, both at school, at martial arts tournaments and at other places. Member will be allowed to participate in supervised and unsupervised training, practice, sparring, and tournaments. Member recognizes such activity as dangerous and assumes all responsibility for any injury or damage that he or she may incur during supervised and unsupervised activity at the school or any other place.

Member will not use Kung Fu, other martial arts, or any other skills that he or she learned at any time other than the purpose of school exhibition, instruction, self-defense or defense of others. In the event he or she causes injury to any person or property because of his or her use of Kung Fu or other martial arts, and such third party brings any legal proceeding or claims against the school, its employees, teachers, instructors and/or management, Member agrees to hold the school its employees, teachers, instructors, and/or management harmless thereon and indemnify the school, its employees, teachers, instructors, and/or management for any loss arising there from.

School shall allow Member to participate, and school shall teach Kung Fu or other martial arts to Member, so long as Member pay membership dues on current basis and complies with the rules of school, and any other rules imposed from hereon. Ranks and Course completion's maybe awarded to the Member in the sole discretion of the school and maybe also be removed in the sole discretion of the school. Chinese Kung-Fu Association reserves the right to refuse service to any person or member at any point in time.
Member agrees not to engage in the teaching or instruction of kung fu or other martial arts by himself or herself or through agents, corporations, partners, or otherwise, in competition with the school unless he or she has expressed written consent to do so
By signing this form, I give up, release and waive all claims and rights that I may have against the school and it's owners, management, employees, teachers, instructors, and members for any injuries or losses I might suffer while enrolled or participating in school activities and classes, including injuries or losses which may be due, in whole or part, to the failure of the school and its owners, management, employees, teachers and instructors to use due care in training, supervision or furnishings of safe equipment or association or which may be due to any other carelessness or recklessness by the school or its owners, management, employees, teachers, instructors, or other members. Member or member’s parent or guardian hereby state that member is free of any heart conditions or problems which would inhibit or prohibit physical exertion and participation in these types of physical and mental activities.
